Complete Nudge Fit intake forms

Receive notification "Approved for Nudge Fit" via text or email within 1 - 2 business days.

Schedule your video telehealth appointment by joining our clinic Spruce Health app.
Step 3
Have your video intake consultation
During the appointment

Review your lab work and medical history with the provider.

Discuss your weight loss goals, exercise and nutrition plan, and treatment options.

Schedule your 1-month follow-up visit at the end of your appointment.
Step 4
Order your medication
After the consultation

Order your prescribed medication through our Nudge Fit store.

We'll submit your order to our compounding pharmacy within 24 - 48 business hours.

What to expect next
Follow-up appointments

Scheduled on a monthly basis during the start of your treatment.

Once you have reached maintenance dose, follow up every 3 months.
Required lab work

All Nudge Fit patients must complete labs within 30 days of starting the program.

Our provider can give you a lab order during your initial consultation if needed.

Already have labs within the last 3 months from another provider? Attach a picture of your lab results to the intake consultation form or send us a copy via Spruce Health app.
